- Emergency Contacts (Completed on website)
- Family Questionnaire (Completed on website)
- Medical Release (Download on website and mail in)
- Tuition Schedule and Agreement (Download on website and mail in)
- ACH Authorization (ACH payments, not needed if already on file)
- Blank Check (ACH tuition payments, not needed if already on file)
- Supply Fee (non-refundable once submitted)
- Tuition Deposit (non-refundable once submitted)
